Instructions for Applications/Evaluation/Reference Forms

All completed forms can be scanned and emailed to borr@fca.org
*If you desire to coach for the So. Cal. Catch please fill out the Coaches Interest Form here or call Ben Orr at (562) 686-8262.
Players and Coaches:
Please read the following information before sending in any form to the So. Cal. Catch:
To Play with the So. Cal. Catch:
► You must currently be a full-time college/university student, having completed a minimum of one semester (12 credit hours)at that school.
► You cannot be a graduating senior.
► NCAA rules state that if a summer collegiate team has a Division I, II, or III coach they cannot coach over junior collegiate players.
We need from each applying player:
► Player Application Form
► Player Evaluation Form from a current baseball coach
► 3 Reference Forms -
- 1 from a personal contact (cannot be a family member)
- 1 Professional
- 1 Pastoral
We need from each coach that wants to coach for the Catch:

► 3 Reference Forms -
- 1 from a personal contact (cannot be a family member)
- 1 Professional
- 1 Pastoral
If you are the coach of an applying player:
